Checklist to get your temporary license

Complete classroom instruction successfully
One week before you turn 15 1/2 get 3001 form from Mr. Gagas
Download, print, complete registration form for Drivers 2 B
Send 3001 form, completed registration form & $225 if paying in full or $125 if doing the payment plan to Drivers 2 B, 644 Hillcrest Dr. Suite #3, Waupaca, WI, 54981
You will receive back in the mail the signed 3001 form and the name and phone number of your driving instructor
Go to D.O.T. to receive your temps. You will need the signed 3001 form, certified birth certificate, social security number, $35, and a parent with you
Schedule 1st drive with instructor
At first drive bring a parent to complete contract with driving school and receive blank completion certificate MV3192
Have Mr. Gagas sign that you have completed classroom instruction
Bring completed MV3192 to second behind the wheel lesson
